Pregnancy in Spain: what is free, what expires

The tests the public system covers, the windows that do not come back, and the deadlines that cost money if you miss them. With the official source under each one.

full — Spain

Tests, exemptions and deadlines, each from the national source.

We are not a local edition: we say that a thing exists and that it has a date, in the words you will need at the counter — then we send you to the body that decides.

What is free, and how to ask for it

  • Pregnancy care is in the common services portfolio

In Spain the monitoring of a normal pregnancy, antenatal education, the birth — epidural included, following each health service's protocols — and the postpartum visit in the first month all sit inside the cartera de servicios comunes of the national health system. They are guaranteed everywhere in the country: there is no charge to pay and no exemption code to show.

  • The floor is national, the rest is regional

That is the floor, guaranteed everywhere. Each autonomous community can add to it — extra scans, screening, programmes of its own — and it differs from one to the next. It works the same way as the Italian regions: the national minimum is not the ceiling, and it is worth checking what your own community offers.

What is different in Spain

  • First ultrasound (dating scan) — Dating at 8-12, then combined screening at 11-13
  • Anomaly (anatomy) scan — Between weeks 18 and 22
  • Glucose curve (75 g OGTT) — Between weeks 24 and 28: O'Sullivan 50 g first, NO fasting
  • Growth ultrasound — Around week 34
  • GBS vaginal-rectal swab — Between weeks 35 and 37

The deadlines that cost money

checkedReminder: about 60 days before the due date

In Spain the leave has gone up to 19 weeks

It is no longer sixteen. After the 2025 reform it is 17 weeks each, non-transferable, plus 2 weeks that can be taken at any point until the child turns 8 — nineteen in all. A single-parent family reaches 32. The first 6 weeks after the birth are compulsory and continuous for both parents. Almost every guide in circulation still shows the old number.

Three months back, and not a day more

Whichever the benefit — birth and childcare, risk during pregnancy — Social Security pays at most 3 months back from the day the application is filed. This is the mistake that actually costs money: taking the leave and leaving the paperwork for later. Those weeks never come back.

The two extra weeks are asked for separately

The 2 additional weeks are not part of the main application: they are requested on their own, at most 15 days before the day you want them to start, and they can be used at any point until the child turns 8. For babies born between 2 August 2024 and 31 July 2025 they can be claimed retroactively from January 2026 — under the same three-month rule.

Registering the birth: 72 hours, or ten days

The hospital notifies the Civil Registry within 72 hours, and that is normally the end of it — you sign from the bed. If it does not happen there the duty falls to the parents and the limit is 10 days; after that it takes a registry procedure to put right. Nothing else — health card, benefits — can be done before the child is registered.

Risk during pregnancy is not sick leave

If the job itself is the risk and there is no other post to move to, this is a benefit of its own: no minimum contribution period, only that you are registered. It runs from the day after work stops, and the paperwork starts with a report from your public-health doctor plus a statement from the employer about the job. The usual rule applies: 3 months backdated at most.
